A straightforward referral route

More than identifying available capacity

The right provider must understand the individual, recognise the risks, communicate openly and deliver what has been agreed. TogetherCare considers regular home care, short-term support, complex packages, overnight care, urgent referrals and continuous staffing.

We would rather explain that more preparation is required than accept prematurely and place the person or wider arrangement at risk.

Referral pathways

Different arrangements, one careful assessment standard

01

Local authority referrals

Planned home care, complex packages, crisis response, supported living or other commissioned arrangements subject to the applicable contract route.

02

NHS and CHC

Continuing Healthcare, discharge, fast-track, end-of-life, rehabilitation and other health-funded packages with clear clinical responsibilities.

03

Case-managed care

Bespoke recruitment, package induction, therapy-led outcomes, reports, MDT review and communication with deputies or solicitors.

04

Private clients

Flexible home support for individuals and families who arrange and fund care directly, with the same assessment and safeguarding standards.

05

Hospital discharge

Short-term or ongoing support planned around equipment, medication, access, staffing and the community professional network.

06

Urgent referrals

Prompt consideration where care has broken down or needs have changed, without bypassing essential safety checks.

Complex and specialist referrals

Defining responsibilities before the first shift

Complex packages may include neurological needs, physical disability, brain or spinal injury, learning disability, mental health, respiratory support, enteral nutrition, continence routines or rehabilitation goals.

Specialist tasks are accepted only where assessment, professional instruction, delegation or authorisation, staff training, competence, clinical oversight and TogetherCare’s regulated scope are all clear.

Our referral process

From enquiry to safe mobilisation

01

Initial review

We confirm the broad requirement, location, timescale, funding route and whether the referral appears to fit our services.

02

Information gathering

Current plans, assessments, medication, risk, equipment, professional contacts and legal arrangements are reviewed.

03

Person-led assessment

We meet the individual wherever practicable and understand preferences, communication, outcomes and home environment.

04

Capacity decision

Operations and, where relevant, appropriate clinical professionals confirm staffing, scope, training and a realistic start date.

05

Mobilisation

Workers are matched, briefed, trained and shadowed; records, contacts, access and contingency arrangements are established.

06

Early review

The plan is checked after support begins and revised with the person and authorised partners as needed.

No automatic acceptanceAn urgent timescale or available worker does not by itself make a package safe. TogetherCare confirms the full requirement before accepting responsibility.

Private client services

Flexible care with professional safeguards

Private funding can offer greater choice over timing, continuity and additional support, but it does not reduce the need for assessment, consent, risk planning and safe staffing.

Designed around prioritiesVisits, extended support, companionship, appointments, travel, overnight care or household routines.
Clear fees and expectationsHours, rates, cancellation, review and payment arrangements are agreed before care begins.
The individual remains centralRelatives may help arrange care, but staff communicate directly with the person and respect their choices.

Communication with referrers

Concise, useful and proportionate

Reporting arrangements are agreed at the start. We provide information relevant to delivery, risks and outcomes rather than simply reproducing every daily note.

Quality, challenge and continuity

A provider should speak up when something is unsafe

Professional challenge

If hours, equipment, guidance or the proposed plan appear insufficient, we raise this with the referrer and explain the evidence. Accepting an unsafe arrangement helps no one.

  • Safeguarding concerns escalated
  • Conflicting plans clarified
  • Missing information requested
  • Changes in need reported

Digital oversight

Care and medication records support operational monitoring, reviews and authorised reporting. Records are factual, respectful and completed at the correct time.

We aim for a consistent core team with trained contingency cover; no provider can guarantee the same worker for every shift.
