We look for a safe way forward before looking for a reason to say no

Referrals do not always arrive neatly packaged. Needs may have changed quickly, a discharge may be delayed, an existing arrangement may be unstable or the full professional information may still be coming together.

Our role is to establish what is known, identify the gaps, assess whether the request fits our regulated scope and determine what staffing, training, equipment, funding and professional input would be required. If we cannot safely accept a package, we will explain why and what would need to change for it to be reconsidered.

Suitable same-day mobilisation can be considered when the essential assessment information is available, funding or authorisation is clear, equipment and medication arrangements are safe, and appropriately skilled staff can be allocated. Clear answers before a package begins

Do you accept packages outside St Helens?

Yes. TogetherCare will consider suitable packages across the North West where a safe local staffing model, reliable contingency and appropriate management oversight can be established. St Helens remains our operational base.

Can you build a dedicated team for one person?

Yes. Larger or specialist packages may require a dedicated team recruited and trained around the individual. The mobilisation timescale will reflect recruitment, checks, training, competency and contingency requirements.

Can care start before every document is finalised?

Sometimes, but only where enough information is available to assess and manage the immediate risks safely. Missing information, responsibilities and timescales must be explicitly recorded and followed up.

Do you provide nurse-led or clinically informed care?

TogetherCare employs nursing expertise for assessment, oversight, training and review. Any regulated or treatment-related activity must remain within TogetherCare’s approved registration at the time of delivery, supported by suitable professional guidance and staff competency.

How should sensitive information be sent?

Begin with a telephone call or a minimal initial email. Our team will confirm the appropriate secure route before identifiable care records, clinical information or other sensitive documents are transferred.

Immediate danger or medical emergency: call 999. TogetherCare is a care provider, not an emergency medical service. Urgent social care can be considered once immediate medical risks have been addressed.
