Our focus is on preventing unnecessary hospital admissions, supporting safe and timely discharges, and maintaining continuity of care during periods of instability.

Our experienced team is able to support a wide range of complex needs, including mental health crisis, behavioural challenges, rapid deterioration in physical health, and breakdown of existing care arrangements. This allows individuals to remain safely in familiar surroundings while maintaining their independence and comfort.

We offer flexible overnight support based on individual needs. For those who require occasional reassurance or minimal support, a sleep-in service provides a carer who is available throughout the night if needed. For individuals with more complex or frequent needs, we provide waking night care, where a carer remains awake and attentive at all times, ready to respond immediately.

Our team can support with a wide range of needs during the night, including assistance with toileting, repositioning, medication, mobility, and general monitoring.
